Loyalty Program Regulations
Whenever capitalized phrases are found in this document, they shall be understood as follows:
- Regulations – these are the regulations of the loyalty program organized by Zakrzewscy Clinic;
- Organizer – Zakrzewscy Clinic Sp. z o.o.;
- Program – the "Closer Means Better" Loyalty Program conducted by the Organizer, based on which the Participant may receive certain benefits (Bonuses) in exchange for certain expenses incurred at Zakrzewscy Clinic in a given calendar year;
- Participant – a natural person with full legal capacity, voluntarily using the services of Zakrzewscy Clinic;
- Bonuses – benefits to which Program Members are entitled, paid for and performed at the expense of the Organizer.
- The Loyalty Program is organized and administered by the Organizer.
- These Regulations are the only binding document describing the rights and obligations of Participants and the Organizer arising from the Program.
- The program is conducted exclusively in the Zakrzewscy Clinic in Katowice, Porcelanowa 23B Street.
- It is not possible to assign a Bonus to any other person. Bonuses can only benefit the Participant.
- Program Participant can be any natural person who is at least 18 years old, has legal capacity and in less than a calendar year (period from January 1 to December 31 inclusive) incurred expenses registered by the Organizer in the amount of not less than PLN 35 000 (in words: thirty five thousand zlotys) gross.
- Expenses incurred by the Patient are set to reset at the end of the year and new expenses are accrued as of January 1 of the following year.
- Participation in the program, and thus fulfillment of the condition described in § 3(1) shall be confirmed by the Organizer in writing (text message and/or e-mail).
- Participation in the program covers the period from the date of receipt by the Participant of the written confirmation referred to in § 3(2), up to and including December 31 of the following calendar year.
- Only after receiving a written confirmation of participation in the Program, the Participant can use the available Bonuses.
- The Participant declares that has read the regulations concerning the "Closer Means Better" Loyalty Program.
- Information about the Program in the form of a leaflet and these Regulations can be received by the User at the e-mail address given to employees at the Organizer’s reception.
- Expenses incurred by the Participant in the Zakrzewscy Clinic are credited only to the Participant and only to the Participant gives the right to use the Bonuses.
- The Participant or any person who has knowledge of infringements of rights may report them by sending an e-mail to the following address: vip@klinikazakrzewscy.pl, in the body of which he shall indicate the type of infringement and the entity that committed it.
- The Participant may not:
- transfer Bonuses to third parties; either for payment or free of charge;
- create another patient/customer account at Zakrzewscy Clinic in order to multiply participation in the Program and receive Bonuses;
- receive any compensation, especially financial compensation, for unused Bonuses;
- transfer the rights to unused Bonuses to a later period, if this period is not the period of participation in the Program.
- Under the Program, it is prohibited for the Participant to act in a manner that would grossly violate these Regulations or applicable law.
- The Organizer cares about the proper operation and quality of Bonuses resulting from the participation in the Program.
- The Organizer reserves the right to block the Participant from participating in the Program in case of:
- breaking the rules of the agreement and regulations;
- taking actions to the detriment of the Organizer;
- providing false data;
- providing by the Participant in the public space, including social media, information that is untrue or violates the good name of the Organizer or is illegal. In such a case, the Organizer will inform the Participant of its decision in writing (text message or e-mail).
- If the Organizer becomes aware of any unlawful activity by the Participant under the Program, the Organizer is obligated to immediately request that the Participant cease such activity.
1. The Program Participant is entitled to the right of priority in arranging a convenient appointment at the Zakrzewscy Clinic, which means the execution of ordered visit within the contractual period of 72 working hours* (Zakrzewscy Clinic’s working hours) from the moment of booking an appointment. The specification of this bonus is shown in the table below:

*the 72 hour appointment does not apply to the package treatment series, which should be scheduled in advance. In case of cosmetology treatments within 72 hours we provide an appointment with a cosmetologist available at that time, if the Program Participant wishes for a treatment to be performed by a particular cosmetologist and there are no free appointments with the selected cosmetologist, the treatment can be performed at a later date.
2. The Participant of the Program is entitled to arrange private transportation (cab, Uber, Bolt, etc.) paid by the Organizer to the previously arranged and confirmed by phone visit in Zakrzewscy Clinic. To use this bonus, the Participant should inform the Organizer about this fact by phone. This transportation is possible only in the areas described in the table below.

3. The Participant is entitled to participate in closed promotional meetings at the invitation of the Organizer. The Organizer reserves the right to organize any number of such meetings during the Program. The Organizer reserves the right to decide whether the invitation to such a meeting will concern the Participant only, or also the accompanying person. The Organizer will inform the Participant of his decision along with the invitation.
The events shall be organised in accordance with the recommendations and guidelines of the sanitary services. In case of a ban on organization, the Organizer may decide to cancel the event.
4. The Participant is entitled to use the birthday treatment during the Program. This means that on the occasion of the Participant’s birthday he/she will receive the possibility of one free treatment, to choose from:
- Hydrafacial MD
- DNA Radiance
- ZO SKIN care treatment
5. The Organizer shall inform the Participant about the right to a free birthday treatment (phone, text message, e-mail), and the treatment should be arranged on a mutually convenient date. The Organizer reserves the right to change the list of treatments available as a part of this bonus. The Organizer will inform Participants in writing (text message, e-mail) about each such change.
§ 6 COMPLAINT PROCEDURE REGARDING THE ACCUMULATION OF POINTS
- Reservations regarding the proper functioning of the Program may be submitted by the Participant in writing via e-mail to recepcja@klinikazakrzewscy.pl.
- The Organizer will consider the complaint within 14 days of its submission, indicating whether the complaint is acknowledged and how intends to handle it, or informing of the lack of grounds for complaint and the reasons for its position.
- If no information on the manner of resolving the complaint is provided within the time limit specified in clause 3, the complaint shall be deemed resolved in favor of the Participant.
- A reply to the complaint shall be sent to the e-mail address provided by the Participant.
§ 7 PROGRAM TERMINATION
- The Organizer may suspend or terminate the "Closer Means Better'' Loyalty Program. The Participant will be informed about such a decision immediately, at least 30 days before the suspension or termination of the Program. This information may also be communicated on the website of Zakrzewscy Clinic www.klinikazakrzewscy.pl.
- Once the Program is terminated, the Bonus will no longer be available.
- The Regulations are available at www.klinikazakrzewscy.pl and at the reception desk of the clinic in Katowice, Porcelanowa 23B Street.
- The Organizer shall change the provisions of the Regulations for important reasons (e.g. amendment of the law) after prior notification of the Participants through publication of the consolidated text of the Regulations at www.klinikazakrzewscy.pl and sending its content to the e-mail address indicated by the Participants. The amended or modified Regulations shall be binding, after fulfillment of other conditions provided by law, only for legal relations arising after the entry into force of the amendments or modifications.
- Changes to the regulations will not in any way affect the acquired rights of the Participants using the Program before the effective date of changes.
- All rights and obligations under these Regulations shall be governed by the Polish law.
