   1.2 The purpose of this Privacy Policy is to inform users, using the resources of the website available at www.klinikazakrzewscy.pl, about the scope and purposes of processing their personal data, categories of recipients of such personal data, the protection measures applied and the rights available to users in relation to the processing of personal data.

   1.3 The Privacy Policy is detailed in the Personal Data Processing Security Policy and information clauses made available to persons whose data are processed in relation to providing medical services or other services within ZAKRZEWSCY CLINIC.

2.1 ZAKRZEWSCY CLINIC shall make every effort to ensure all the measures, physical, technical and organizational protection of personal data against their accidental or intentional destruction, accidental loss, alteration, unauthorized disclosure, use or access in accordance with generally applicable law in this respect. If personal data is shared with other entities, it is carried out in a safe, contractually secured manner and in accordance with applicable law.

2.2 Users’ personal data shall be processed only to the minimum extent necessary for the purposes for which they are collected.

2.3 ZAKRZEWSCY CLINIC shall make every effort to ensure that the personal data are up-to-date and correct, and shall immediately undertake measures to fulfil requests for rectification or updating of the data.

   5.1 The period for which personal data is stored shall be determined in accordance with applicable law. Moreover, data subjects have the right to be informed about the expected duration of personal data processing.

   6.2 In some cases, personal data may also be disclosed to other data controllers, in accordance with applicable laws.

   6.3 ZAKRZEWSCY CLINIC may make personal data available to other recipients, i.e. processors providing services on behalf of ZAKRZEWSCY CLINIC within the scope of activities requiring data processing, in particular within the scope of courier services, IT, accounting, legal, marketing, market research, organization and execution of events and auditors.

7.1 The www.klinikazakrzewscy.pl uses information stored by means of cookies.

0. 8.1 The User has the right to:

   (a) withdraw consent to the processing of personal data at any time where consent is the legal basis for the processing;

   (b) access to the data and the right to obtain confirmation whether the personal data are processed by ZAKRZEWSCY CLINIC and in what way;

   (c) rectification of outdated or inaccurate personal data, as well as the right to have it completed if incomplete;

   (d) object to the processing of personal data;

   (e) deletion of personal data (the so-called "right to be forgotten"), which can be exercised by the User by directing ZAKRZEWSCY CLINIC to demand immediate deletion of personal data;

   (f) restrict the processing of personal data, by temporarily blocking access to data;

   (g) file a complaint with the supervisory authority, the President of the Office for Personal Data Protection, if the User considers that the processing of his personal data breaches the regulations.

   8.2 ZAKRZEWSCY CLINIC shall carry out the User’s requests without undue delay, but no later than within one month from the date of receipt. This time limit may be extended due to the complexity of the request or the number of requests, by another two months, of which the User shall be informed.

   8.3 As far as the User submits requests to ZAKRZEWSCY CLINIC, it is necessary to give his name and surname and contact data (telephone number or e-mail address, in case of written requests also address of residence). In order to execute some requests it may be necessary to further confirm the identity of the User.

1. 9.1 ZAKRZEWSCY CLINIC shall take technical and organizational measures to protect personal data against unlawful or unauthorized access or use, as well as against accidental destruction, loss or compromise of their integrity by ensuring their confidentiality, integrity, accountability and availability at an appropriate level.

   9.2 Personal data of the users can be processed by third parties only if such an entity undertakes to provide appropriate technical and organizational means guaranteeing security of personal data processing as well as to maintain confidentiality of these data. Each ZAKRZEWSCY CLINIC employee having access to personal data has appropriate authorization and is obliged to maintain confidentiality.

   9.3 Users’ personal data processed within the www.klinikazakrzewscy.pl are encrypted and protected by SSL certificate.

10.1 The User’s consent to the processing of personal data for marketing and commercial purposes, including receipt of the Newsletter, is granted by the so-called "explicit action," e.g. resulting from the fact that the User voluntarily leaves his data and requests to be contacted, sending a query within the scope specified in the consent or by ticking a checkbox.
